Print the form, complete and sign it. Then mail it to the Board of Elections at: P.O. Box 99, Schoharie, NY 12157.

All first time voters who registered by mail must present identification at the polls, either a current photo ID or a copy of a utility bill, paycheck, government check, bank statement or government document showing the name and address of the voter. Voters may also register at the DMV.
